f7143dca3f Fix typo in nsupdate where covers would be equal to type
There was an apparent typo where rdatalist->covers would be assigned the
same value as rdatalist->type.  As nsupdate can't update signatures, the
covers must be dns_rdatatype_none.

f7e5c1db38 Change the 'isc_g_mctx' to be always available
This required couple of internal changes to the isc_mem_debugging.

The isc_mem_debugging is now internal to isc_mem unit and there are
three new functions:

1. isc_mem_setdebugging() can change the debugging setting for an
   individual memory context.  This is need for the memory contexts used
   for OpenSSL, libxml and libuv accounting as recording and tracing
   memory is broken there.

2. isc_mem_debugon() / isc_mem_debugoff() can be used to change default
   memory debugging flags as well as debugging flags for isc_g_mctx.

Additionally, the memory debugging is inconsistent across the code-base.
For now, we are keeping the existing flags, but three new environment
variables have been added 'ISC_MEM_DEBUGRECORD', 'ISC_MEM_DEBUGTRACE'
and 'ISC_MEM_DEBUGUSAGE' to set the global debugging flags at any
program using the memory contexts.

74726b3313 Add and use global memory context called isc_g_mctx
Instead of having individual memory contexts scattered across different
files and called different names, add a single memory context called
isc_g_mctx that replaces named_g_mctx and various other global memory
contexts in various utilities and tests.

f6aed602f0 Refactor the network manager to be a singleton
There is only a single network manager running on top of the loop
manager (except for tests).  Refactor the network manager to be a
singleton (a single instance) and change the unit tests, so that the
shorter read timeouts apply only to a specific handle, not the whole
extra 'connect_nm' network manager instance.

b8d00e2e18 Change the loopmgr to be singleton
All the applications built on top of the loop manager were required to
create just a single instance of the loop manager.  Refactor the loop
manager to not expose this instance to the callers and keep the loop
manager object internal to the isc_loop compilation unit.

This significantly simplifies a number of data structures and calls to
the isc_loop API.

dd9a685f4a simplify code around isc_mem_put() and isc_mem_free()
it isn't necessary to set a pointer to NULL after calling
isc_mem_put() or isc_mem_free(), because those macros take
care of it automatically.

8487e43ad9 make all ISC_LIST_FOREACH calls safe
previously, ISC_LIST_FOREACH and ISC_LIST_FOREACH_SAFE were
two separate macros, with the _SAFE version allowing entries
to be unlinked during the loop. ISC_LIST_FOREACH is now also
safe, and the separate _SAFE macro has been removed.

similarly, the ISC_LIST_FOREACH_REV macro is now safe, and
ISC_LIST_FOREACH_REV_SAFE has also been removed.

74a8acdc8d Separate the single setter/getter functions for TCP timeouts
Previously all kinds of TCP timeouts had a single getter and setter
functions. Separate each timeout to its own getter/setter functions,
because in majority of cases only one is required at a time, and it's
not optimal expanding those functions every time a new timeout value
is implemented.

70ad94257d Implement tcp-primaries-timeout
The new 'tcp-primaries-timeout' configuration option works the same way
as the existing 'tcp-initial-timeout' option, but applies only to the
TCP connections made to the primary servers, so that the timeout value
can be set separately for them. The default is 15 seconds.

Also, while accommodating zone.c's code to support the new option, make
a light refactoring with the way UDP timeouts are calculated by using
definitions instead of hardcoded values.

522ca7bb54 switch to ISC_LIST_FOREACH everywhere
the pattern `for (x = ISC_LIST_HEAD(...); x != NULL; ISC_LIST_NEXT(...)`
has been changed to `ISC_LIST_FOREACH` throughout BIND, except in a few
cases where the change would be excessively complex.

in most cases this was a straightforward change. in some places,
however, the list element variable was referenced after the loop
ended, and the code was refactored to avoid this necessity.

also, because `ISC_LIST_FOREACH` uses typeof(list.head) to declare
the list elements, compilation failures can occur if the list object
has a `const` qualifier.  some `const` qualifiers have been removed
from function parameters to avoid this problem, and where that was not
possible, `UNCONST` was used.

5cff8f9017 implicitly declare list elements in ISC_LIST_FOREACH macros
ISC_LIST_FOREACH and related macros now use 'typeof(list.head)' to
declare the list elements automatically; the caller no longer needs
to do so.

ISC_LIST_FOREACH_SAFE also now implicitly declares its own 'next'
pointer, so it only needs three parameters instead of four.

The libedit is now ubiquitous and has a licences compatible with
MPL 2.0.  Drop readline (GPL 3.0) and editline (obsolete) support
in favor of libedit.

2f27d66450 Refactor to use list-like macro for message sections
In the code base it is very common to iterate over all names in a message
section and all rdatasets for each name, but various idioms are used for
iteration.

This commit standardizes them as much as possible to a single idiom,
using the macro MSG_SECTION_FOREACH, similar to the existing
ISC_LIST_FOREACH.

initialize the objects after retrieving them. this is no longer
necessary.

afb424c9b6 simplify dns_name_fromtext() interface
previously, dns_name_fromtext() took both a target name and an
optional target buffer parameter, which could override the name's
dedicated buffer. this interface is unnecessarily complex.

we now have two functions, dns_name_fromtext() to convert text
into a dns_name that has a dedicated buffer, and dns_name_wirefromtext()
to convert text into uncompressed DNS wire format and append it to a
target buffer.

in cases where it really is necessary to have both, we can use
dns_name_fromtext() to load the dns_name, then dns_name_towire()
to append the wire format to the target buffer.

cf098cf10d avoid the 'target' buffer in dns_name_fromtext()
dns_name_fromtext() stores the converted name in the 'name'
passed to it, and optionally also copies it in wire format to
a buffer 'target'. this makes the interface unnecessarily
complex, and could be simplified by having a different function
for each purpose. as a first step, remove uses of the target
buffer in calls to dns_name_fromtext() where it wasn't actually
needed.

08e966df82 Remove offsets from the dns_name and dns_fixedname structures
The offsets were meant to speed-up the repeated dns_name operations, but
it was experimentally proven that there's actually no real-world
benefit.  Remove the offsets and labels fields from the dns_name and the
static offsets fields to save 128 bytes from the fixedname in favor of
calculating labels and offsets only when needed.

f5c204ac3e Move the library init and shutdown to executables
Instead of relying on unreliable order of execution of the library
constructors and destructors, move them to individual binaries.  The
advantage is that the execution time and order will remain constant and
will not depend on the dynamic load dependency solver.

This requires more work, but that was mitigated by a simple requirement,
any executable using libisc and libdns, must include <isc/lib.h> and
<dns/lib.h> respectively (in this particular order).  In turn, these two
headers must not be included from within any library as they contain
inlined functions marked with constructor/destructor attributes.

10accd6260 clean up uses of ISC_R_NOMEMORY
the isc_mem allocation functions can no longer fail; as a result,
ISC_R_NOMEMORY is now rarely used: only when an external library
such as libjson-c or libfstrm could return NULL. (even in
these cases, arguably we should assert rather than returning
ISC_R_NOMEMORY.)

code and comments that mentioned ISC_R_NOMEMORY have been
cleaned up, and the following functions have been changed to
type void, since (in most cases) the only value they could
return was ISC_R_SUCCESS:

- dns_dns64_create()
- dns_dyndb_create()
- dns_ipkeylist_resize()
- dns_kasp_create()
- dns_kasp_key_create()
- dns_keystore_create()

(the exception is dns_view_create(), which could have returned
other error codes in the event of a crypto library failure when
calling isc_file_sanitize(), but that should be a RUNTIME_CHECK
anyway.)

64ffbe82c0 Separate the connect and the read timeouts in dispatch
The network manager layer has two different timers with their
own timeout values for TCP connections: connect timeout and read
timeout. Separate the connect and the read TCP timeouts in the
dispatch module too.

679e90a57d Add isc_log_createandusechannel() function to simplify usage
The new
isc_log_createandusechannel() function combines following calls:

    isc_log_createchannel()
    isc_log_usechannel()

calls into a single call that cannot fail and therefore can be used in
places where we know this cannot fail thus simplifying the error
handling.

091d738c72 Convert all categories and modules into static lists
Remove the complicated mechanism that could be (in theory) used by
external libraries to register new categories and modules with
statically defined lists in <isc/log.h>.  This is similar to what we
have done for <isc/result.h> result codes.  All the libraries are now
internal to BIND 9, so we don't need to provide a mechanism to register
extra categories and modules.

b2dda86254 Replace isc_log_create/destroy with isc_logconfig_get()
Add isc_logconfig_get() function to get the current logconfig and use
the getter to replace most of the little dancing around setting up
logging in the tools. Thus:

    isc_log_create(mctx, &lctx, &logconfig);
    isc_log_setcontext(lctx);
    dns_log_setcontext(lctx);
    ...
    ...use lcfg...
    ...
    isc_log_destroy();

is now only:

    logconfig = isc_logconfig_get(lctx);
    ...use lcfg...

For thread-safety, isc_logconfig_get() should be surrounded by RCU read
lock, but since we never use isc_logconfig_get() in threaded context,
the only place where it is actually used (but not really needed) is
named_log_init().

e6f2f2a5e6 Initialize the DST subsystem implicitly
Instead of calling dst_lib_init() and dst_lib_destroy() explicitly by
all the programs, create a separate memory context for the DST subsystem
and use the library constructor and destructor to initialize the DST
internals.

63659e2e3a complete removal of isc_loop_current()
isc_loop() can now take its place.

This also requires changes to the test harness - instead of running the
setup and teardown outside of th main loop, we now schedule the setup
and teardown to run on the loop (via isc_loop_setup() and
isc_loop_teardown()) - this is needed because the new the isc_loop()
call has to be run on the active event loop, but previously the
isc_loop_current() (and the variants like isc_loop_main()) would work
even outside of the loop because it needed just isc_tid() to work, but
not the full loop (which was mainly true for the main thread).

0c18ed7ec6 Remove isc__tls_setfatalmode() function and the calls
With _exit() instead of exit() in place, we don't need
isc__tls_setfatalmode() mechanism as the atexit() calls will not be
executed including OpenSSL atexit hooks.

4bec711fe3 Use _exit() in the fatal() function
Since the fatal() isn't a correct but rather abrupt termination of the
program, we want to skip the various atexit() calls because not all
memory might be freed during fatal() call, etc.  Using _exit() instead
of exit() has this effect - the program will end, but no destructors or
atexit routines will be called.

f5af981831 Change dns_message_create() function to accept memory pools
Instead of creating new memory pools for each new dns_message, change
dns_message_create() method to optionally accept externally created
dns_fixedname_t and dns_rdataset_t memory pools.  This allows us to
preallocate the memory pools in ns_client and dns_resolver units for the
lifetime of dns_resolver_t and ns_clientmgr_t.

c9b4b45943 Replace the linked list of TCP dispatches with hash table
Reusing TCP connections with dns_dispatch_gettcp() used linear linked
list to lookup existing outgoing TCP connections that could be reused.
Replace the linked list with per-loop cds_lfht hashtable to speedup the
lookups.  We use cds_lfht because it allows non-unique node insertion
that we need to check for dispatches in different connection states.

6fd06c461b Make dns_dispatch bound to threads
Instead of high number of dispatches (4 * named_g_udpdisp)[1], make the
dispatches bound to threads and make dns_dispatchset_t create a dispatch
for each thread (event loop).

This required couple of other changes:

1. The dns_dispatch_createudp() must be called on loop, so the isc_tid()
   is already initialized - changes to nsupdate and mdig were required.

2. The dns_requestmgr had only a single dispatch per v4 and v6.  Instead
   of using single dispatch, use dns_dispatchset_t for each protocol -
   this is same as dns_resolver.

4ca64c1799 Pin dns_request to the associated loop
When dns_request was canceled via dns_requestmgr_shutdown() the cancel
event would be propagated on different loop (loop 0) than the loop where
request was created on.  In turn this would propagate down to isc_netmgr
where we require all the events to be called from the matching isc_loop.

Pin the dns_requests to the loops and ensure that all the events are
called on the associated loop.  This in turn allows us to remove the
hashed locks on the requests and change the single .requests list to be
a per-loop list for the request accounting.

Additionally, do some extra cleanup because some race condititions are
now not possible as all events on the dns_request are serialized.

- remove an unnecessary call to dns_tkeyctx_create() in ns_server_create()
  (the TKEY context that was created there would soon be destroyed and
  another one created when the configuration was loaded).

6105a7d360 convert TSIG keyring storage from RBT to hash table
since it is not necessary to find partial matches when looking
up names in a TSIG keyring, we can use a hash table instead of
an RBT to store them.

the tsigkey object now stores the key name as a dns_fixedname
rather than allocating memory for it.

the `name` parameter to dns_tsigkeyring_add() has been removed;
it was unneeded since the tsigkey object already contains a copy
of the name.

the opportunistic cleanup_ring() function has been removed;
it was only slowing down lookups.

ffacf0aec6 use algorithm number instead of name to create TSIG keys
the prior practice of passing a dns_name containing the
expanded name of an algorithm to dns_tsigkey_create() and
dns_tsigkey_createfromkey() is unnecessarily cumbersome;
we can now pass the algorithm number instead.

a6e187a8d5 further dns_tsigkey API tweaks
- remove the 'ring' parameter from dns_tsigkey_createfromkey(),
  and use dns_tsigkeyring_add() to add key objects to a keyring instead.
- add a magic number to dns_tsigkeyring_t
- change dns_tsigkeyring_dumpanddetach() to dns_tsigkeyring_dump();
  we now call dns_tsigkeyring_detach() separately.
- remove 'maxgenerated' from dns_tsigkeyring_t since it never changes.

6fa8524bba use ISC_REFCOUNT_IMPL for dns_tsigkey and dns_tsigkeyring
use the ISC_REFCOUNT attach/detach implementation in dns/tsig.c
so that detailed tracing can be used during refactoring.

dns_tsig_keyring_t has been renamed dns_tsigkeyring_t so the type
and the attach/detach function names will match.
