Add support for setting thread stack size
When running the isc_quota unit test with less than usual amount of
RAM (e.g. in a CI for architectures with 32 bits of address space),
the pthread_create() function fails with the "Resource temporarily
unavailable (11):" error code.

Add functions to get and set the thread stack size (if requested),
and use these to set the thread stack size to smaller value in the
isc_quota unit test.

Change the 'isc_g_mctx' to be always available
This required couple of internal changes to the isc_mem_debugging.

The isc_mem_debugging is now internal to isc_mem unit and there are
three new functions:

1. isc_mem_setdebugging() can change the debugging setting for an
individual memory context. This is need for the memory contexts used
for OpenSSL, libxml and libuv accounting as recording and tracing
   memory is broken there.

2. isc_mem_debugon() / isc_mem_debugoff() can be used to change default
memory debugging flags as well as debugging flags for isc_g_mctx.

Additionally, the memory debugging is inconsistent across the code-base.
For now, we are keeping the existing flags, but three new environment
variables have been added 'ISC_MEM_DEBUGRECORD', 'ISC_MEM_DEBUGTRACE'
and 'ISC_MEM_DEBUGUSAGE' to set the global debugging flags at any
program using the memory contexts.

Add and use global memory context called isc_g_mctx
Instead of having individual memory contexts scattered across different
files and called different names, add a single memory context called
isc_g_mctx that replaces named_g_mctx and various other global memory
contexts in various utilities and tests.

Give every memory pool a name
Instead of giving the memory pools names with an explicit call to
isc_mempool_setname(), add the name to isc_mempool_create() call to have
all the memory pools an unconditional name.

dd9a685f4a simplify code around isc_mem_put() and isc_mem_free()
it isn't necessary to set a pointer to NULL after calling
isc_mem_put() or isc_mem_free(), because those macros take
care of it automatically.

b7ae514b3b fix out-of-tree mem_test
Previously changed mem_test (!10320) introduces a test which checks for
the value of `__FILE__`, which is different if the build is done
out-of-tree or not, even though this is not relevant for the test (only
the base filename is). This result in a broken test for out-of-tree
builds. Fix this by changing the way the "grep" is done in the test,
ignoring the optional path prefix in the filename.

4eb2cd364a copy __FILE__ when allocating memory
When allocating memory under -m trace|record, the __FILE__ pointer is
stored, so it can be printed out later in order to figure out in which
file an allocation leaked. (among others, like the line number).

However named crashes when called with -m record and using a plugin
leaking memory. The reason is that plugins are unloaded earlier than
when the leaked allocations are dumped (obviously, as it's done as late
as possible). In such circumstances, __FILE__ is dangling because the
dynamically loaded library (the plugin) is not in memory anymore.

Fix the crash by systematically copying the __FILE__ string
instead of copying the pointer. Of course, this make each allocation to
consume a bit more memory (and longer, as it needs to calculate the
length of __FILE__) but this occurs only under -m trace|record debugging
flags.

In term of unit test, because grepping in C is not fun, and because the
whole "syntax" of the dump output is tested in other tests, this simply
search for a substring in the whole buffer to make sure the expected
allocations are found.

Replace isc_mem_destroy() with isc_mem_detach()
Remove legacy isc_mem_destroy() and just use isc_mem_detach() as
isc_mem_destroy() doesn't play well with call_rcu API.

Move the library init and shutdown to executables
Instead of relying on unreliable order of execution of the library
constructors and destructors, move them to individual binaries. The
advantage is that the execution time and order will remain constant and
will not depend on the dynamic load dependency solver.

This requires more work, but that was mitigated by a simple requirement,
any executable using libisc and libdns, must include <isc/lib.h> and
<dns/lib.h> respectively (in this particular order). In turn, these two
headers must not be included from within any library as they contain
inlined functions marked with constructor/destructor attributes.

Add isc_mem_overmem unit test
The new unit isc_mem_overmem unit test sets hi and lo water marks and
then does allocations to go over:

0. x < lo_water
1. lo_water < x < hi_water
2. x > hi_water
3. lo_water < x < hi_water
4. < lo_water

Remove ISC_MEM_ZERO and isc_mem_*x() API
Use the new isc_mem_c*() calloc-like API for allocations that are
zeroed.

In turn, this also fixes couple of incorrect usage of the ISC_MEM_ZERO
for structures that need to be zeroed explicitly.

There are few places where isc_mem_cput() is used on structures with a
flexible member (or similar).

Remove ISC_MEM_ALIGN() memory flag
The ISC_MEM_ALIGN() was not used anywhere (except mem.c itself), so just
remove the unused flag.

Simplify isc_thread a little
Remove the `isc_threadarg_t` and `isc_threadresult_t`
typedefs which were unhelpful disguises for `void *`,
and free the dummy jemalloc allocation sooner.

Refactor isc_time_now() to return time, and not result
The isc_time_now() and isc_time_now_hires() were used inconsistently
through the code - either with status check, or without status check,
or via TIME_NOW() macro with RUNTIME_CHECK() on failure.

Refactor the isc_time_now() and isc_time_now_hires() to always fail when
getting current time has failed, and return the isc_time_t value as
return value instead of passing the pointer to result in the argument.

3d4e41d076 Remove the total memory counter
The total memory counter had again little or no meaning when we removed
the internal memory allocator. It was just a monotonic counter that
would count add the allocation sizes but never subtracted anything, so
it would be just a "big number".

Replace isc_mem_*_aligned(..., alignment) with isc_mem_*x(..., flags)
Previously, the isc_mem_get_aligned() and friends took alignment size as
one of the arguments. Replace the specific function with more generic
extended variant that now accepts ISC_MEM_ALIGN(alignment) for aligned
allocations and ISC_MEM_ZERO for allocations that zeroes
the (re-)allocated memory before returning the pointer to the caller.

Make the debugging flags local to the memory context
Previously, the isc_mem_debugging would be single global variable that
would affect the behavior of the memory context whenever it would be
changed which could be after some allocation were already done.

Change the memory debugging options to be local to the memory context
and immutable, so all allocations within the same memory context are
treated the same.

2c3b2dabe9 Move all the unit tests to /tests/<libname>/
The unit tests are now using a common base, which means that
lib/dns/tests/ code now has to include lib/isc/include/isc/test.h and
link with lib/isc/test.c and lib/ns/tests has to include both libisc and
libdns parts.

Instead of cross-linking code between the directories, move the
/lib/<foo>/test.c to /tests/<foo>.c and /lib/<foo>/include/<foo>test.h
to /tests/include/tests/<foo>.h and create a single libtest.la
convenience library in /tests/.

At the same time, move the /lib/<foo>/tests/ to /tests/<foo>/ (but keep
it symlinked to the old location) and adjust paths accordingly. In few
places, we are now using absolute paths instead of relative paths,
because the directory level has changed. By moving the directories
under the /tests/ directory, the test-related code is kept in a single
place and we can avoid referencing files between libns->libdns->libisc
which is unhealthy because they live in a separate Makefile-space.

In the future, the /bin/tests/ should be merged to /tests/ and symlink
kept, and the /fuzz/ directory moved to /tests/fuzz/.
