Remove and fix stray Hicontrast (HC) icons. - Remove old HC icons from Human, Oxygen & Tango themes. (They should only exist in specific HC theme). - Move few icons misplaced in Galaxy to correct HC theme. - Fix HC filename: lch_autopilotmenu > lc_autopilotmenu.
